SMS-Text Messaging Terms & Conditions
Practice: Smiles On Church (“we,” “us,” or “our”)
1. Program Description
By opting into our SMS/text messaging service, you authorize Smiles On Church to send you text messages at the mobile number you provided, using a 10-digit long code (10DLC) or other approved texting number. These messages may include appointment confirmations, reminders, rescheduling notices or cancellations, account or billing notices, follow-up instructions, practice updates, and offers or promotions (depending on your opt-in status).
2. Consent & Opt-In
- You may opt in by providing your mobile number and checking the consent box where indicated (or via another express method) allowing our practice to send you SMS/text messages.
- Your consent is not a condition of receiving dental services.
- By opting in, you confirm you are the owner or authorized user of the mobile number you provided.
3. Types of Messages & Frequency
- Message types include, but are not limited to: appointment reminders and confirmations, cancellations/reschedules, treatment follow-ups, account/billing notifications, practice announcements, and, if you’ve separately opted in, promotional messages.
- Message frequency varies based on your interaction with our practice and scheduled treatments. Reminder messages are typically sent 1–2 days before appointments.
- Standard message and data rates may apply according to your carrier’s plan.
4. Opt-Out / Unsubscribe
You may opt out of future SMS messages at any time by replying STOP to any message you receive from us. Reply HELP for assistance. After opting out of promotional messages, you may still receive important transactional or practice-related messages unless you withdraw from all SMS communications.
5. Message and Carrier Disclaimer & Liability
- Message delivery is subject to your mobile carrier’s network and device; we do not guarantee receipt of any message.
- You are responsible for any charges associated with receiving SMS from us.
- We are not liable for delays, failures in delivery, or third-party interception of messages. Use of the service is at your own risk.
- We may modify, suspend, or terminate the SMS program or these Terms at any time; changes will be posted on our website, and continued use constitutes acceptance of the revised terms.
6. Eligibility & Number Changes
- You must be at least 18 years old, or have parental/guardian consent, to enroll.
- If your mobile number changes or is reassigned, please update us promptly — we are not responsible for messages sent to an outdated or reassigned number.
7. Additional Terms
These SMS Terms & Conditions are part of our overall Terms of Service and Privacy Policy; to the extent of any conflict, these SMS Terms control as to SMS communications.
